Case Management
Case Management is a vital link between persons living with HIV/AIDS and the services they need to remain adherent with medical and other necessary treatment available in the community. Friends For Life’s Case Management services are funded by the Ryan White Care Act passed through the Shelby County Government.
Case Management . . .
- Helps persons who are living with HIV/AIDS find and receive services to address their medical, mental and other personal needs
- Helps eliminate barriers that make it difficult for persons living with HIV/AIDS to receive the services they need
- Services are provided by professionals who have extensive experience with persons living with HIV/AIDS
- Completes an individualized assessment with each person to determine his/her service needs
- Develops a Plan of Action with each person to address his/her needs
Refers people to:
Medical treatment
Mental health care
Alcohol and drug abuse treatment
Legal services
Homeless service provider system
All services offered by Friends For Life
- Helps people living with HIV learn how to navigate the community’s service system
- Provides a listening ear to people in need
- Helps people living with HIV manage depression, isolation and low self-esteem
- Continues working until a resolution to problem is found
- Helps people until they can help themselves
