Case Management
The goal of Case Management is to identify persons with HIV/AIDS who are not compliant with medical treatment and provide them with case management services that eliminate the barriers to medical treatment.
The focus of the Friends For Life Case Manager is establishing linkages with the mental health system, homeless service provider system, alcohol and drug abuse treatment system, and corrections system in order to facilitate the client’s adherence to medical treatment. The Case Manager will also follow up with existing clients in order to link them to the services above and to inform them about services being offered both in our agency and in the community at large.
A staff member fluent in Spanish is available upon request to better serve the growing Latino population.
The activities of the Friends For Life case managers are coordinated with other agencies, including the Adult Special Care Center. The Adult Special Care Center Outreach Case Managers will focus their efforts on those individuals who are already patients of the Adult Special Care Center but who are noncompliant with medical treatment from the Center. These outreach workers will be stationed onsite at Friends For Life during food pantry hours to contact patients of the Adult Special Care Center.
